MBD Care is a dietitian-led healthcare organization connecting NYC Medicaid members with free nutrition services — home-delivered meals, weekly groceries, and nutrition counseling — through New York State's Social Care Network program. We're growing our Brooklyn team.

Social Care Navigator
This is a phone-and-computer care coordination role — no field work, no home visits. You'll call Medicaid members referred to us, complete social needs screenings, and connect eligible New Yorkers with free meal deliveries, groceries, and nutrition counseling. Full training provided, including our care coordination platform (Unite Us).
What We're Looking For
A warm, patient, persistent phone presence. Bilingual strongly preferred — Russian, Spanish, Mandarin, Cantonese, or Ukrainian earns premium pay. Organized and detail-oriented, comfortable learning new software, with a quiet workspace and reliable internet for remote days. Call center, social services, healthcare, or insurance experience is a plus. High school diploma required; offer contingent on federal and state healthcare exclusion-list screening.
